The Proof of Claim form details the information needed for the surety claims department to initiate an investigation of a general surety bond claim. Once completed, mail a printed copy of the Proof of Claim form and claim documentation to the surety claims department or submit a scanned version via email. Claimants can also contact the surety claims department via telephone, fax and email at the information below for the purpose of submitting claims.
