For your convenience there are two easy options for completing the Notary Application and State of Florida Bond of Notary Public form.

The Online Notary Application allows you to type in the required information while online. The online Price List will automatically calculate the amount owed for your notary items selected along with the mandatory 1% Florida ordered Emergency Assessment on premium.

The Printable Notary Application is also available if you wish to complete the application by hand. If you choose to use the Printable Notary Application and Price List you will need to manually calculate the amount owed for your notary items selected along with the 1% Florida Department of Insurance Emergency Assessment on premium.

Training: An individual must complete the Florida required notary training course prior to applying to become a Florida Notary of Public. The original training certificate must be submitted with the Notary Application. There are two formats available for the training course. Online via the internet or an instructor based class. The online course can be found at the following websites; http://notaries.dos.state.fl.us or www.notaryeducation.com. If you would like to attend the training class in person you can find the nearest location at www.notaryeducation.com.

Proper Identification: The state of Florida requires applicants to provide a Social Security Number and Florida Drivers License Number as form of identification. If you do not have a Florida Driver’s License please provide a photocopy of one of the following: Florida Voter’s Registration Card, Florida I.D. Card, Homestead Exemption, or a Sworn Affidavit of Florida Residency.

Declaration of Domicile: If you are not a United States citizen, you must obtain a recorded Declaration of Domicile from your county courthouse. This is the only acceptable form.

Affidavit of Character: An individual who is not a relative, and who has known you for one or more years must complete the Affidavit of Character section of the application. The affidavit must be signed and the name of the affiant must be printed or typed underneath the signature.

Oath of Office: Please be sure to sign your legal signature, print or type your name, and include the current date.

State of Florida Bond of Notary Public: Fill in your name on the line indicated (Name of Applicant), sign the Bond Form on the line next to the large X, and date the form directly underneath your signature. We will complete the remainder of the form.

You will receive your notary commission and supplies in about 2-3 weeks from the date submitted, provided the application is complete and pending state approval.

Payment: Please make your check payable to Accredited Group Agency, Inc. and mail with your application, bond form and original training certificate to the address listed below. Payment by credit card is also available. Provide credit card information and original signature on Price List form.

The state of Florida placed a mandatory 1% Emergency Assessment on certain lines of insurance premium effective January 1, 2007. As an insurance company, Accredited is required to collect this 1% Emergency Assessment on the premium for Notary bonds and on the Errors and Omissions Insurance if that option is purchased. The 1% Emergency Assessment must be included with your payment. To calculate the 1% assessment owed, multiple the premium amounts by .01. Example: (1) Notary Bond is $30.00 premium. 30 X .01 = $0.30(assessment). Example: $10,000 Errors & Omissions policy is $40.00 premium. 40 X .01 = $0.40 (assessment)

Online Application and State of Florida Bond of Notary Public Form: You may complete the Online Application, Bond form and Price List online by typing the required information into the fields. All information must be provided. Do not leave any blanks on the application. Enter the word "none" if you do not have a business address, or for any questions which do not apply to you. A physical address must be provided. P.O. Box numbers are not accepted by the State of Florida unless a rural route number is provided. Print out the completed Online Application, Bond Form and Price List. Place your signature by the X where requested on the application, Bond form, and on the Price List if paying by credit card. . Mail the signed original Application, the Bond form, the Price List, and the original training certificate along with payment to the address provided.

Printable Application: Print out the Printable Notary Application, the State of Florida Bond of Notary Public form and the Price List. All information must be provided.
